Guiding steps for filling the claim form for payment of Perks & Allowances for the period 26.11.2008 to 04.10.2009 or part thereof in case of separated executives of SAIL Plants/Units

The claimant is required to opt for ‘Live case’ or ‘Death case’, as applicable, on the log-in page

For log-in PAN No. and Date of Birth (DOB) of the executive in ‘Live case’ as well as ‘Death case’ is to be entered.

Contact no. and e-mail ID is mandatory in all the cases.

The fields marked * are mandatory.

If the information/documents (including MIN No.) required in non-mandatory fields are available, the same may also be provided.

All the fields of Bank A/c details i.e. A/c No., IFSC, Bank name and Branch are mandatory

All the documents should be self-certified.

Documents can be uploaded upto 200kb. Uploaded documents can be replaced/reloaded before final ‘SUBMIT’ of claim form.

After keying-in all the details, print-out of the Form is required to be taken and duly signed Form is to be uploaded on the same webpage at SAIL Website and to be submitted.
